The Blue Value Primary Care Model, commonly referred to as Patient Centered Medical Home (PCMH), is a voluntary primary care alternative payment model for health care delivery in which the primary care provider is financially compensated through participation and care coordination, chronic disease management, and quality outcomes for Blue Cross Blue Shield attributed members.
