Getting Your Form 1095-B

You have 4 ways to ask for your BCBSMT Form 1095-B. Once we get your request, we’ll get your Form 1095-B mailed to you within 30 days.

Online

Blue Access for Members^SM
Sign up or log in to your secure online account, then visit My Account and select Forms and Documents to download your Form 1095-B.

Phone

Call the customer service number listed on your BCBSMT member ID card to ask for your Form 1095-B to be mailed to your address on file, or call 1-855-710-6984.

Call Us

Mail

Fill out the 1095-B request form and mail it to the address listed on the form.

Based on your health plan, your employer may have sent you Form 1095-C.
